International Example Learning Journey

Inspired by the kinds of questions we receive from readers outside the United States, including Monaco. This is a fictional, composite scenario. It identifies no real person.

Not for emergencies. These educational resources cannot help in an emergency. If you have chest pain, trouble breathing, fainting, stroke symptoms, or think you may be having a medical emergency, contact your local emergency number now (for example 112 in Europe including Monaco, 911 in North America, 999 in the UK).

This scenario does not choose a therapy, set an LDL target, or advise anyone. It shows the kinds of topics a person might learn about and questions they might bring to their own cardiologist.

The scenario

An adult living in Europe has had a coronary calcium scan reported in the 100–299 range, already takes a statin prescribed by their own clinician, and has read about newer PCSK9 therapies. They want to understand the science before their next appointment.

  1. 1

    Understand the score

Learn what a CAC score in the 100–299 range generally means at a population level, and why it is not a personal prediction.

  2. 2

    See the whole picture

Review the risk concepts (LDL-C, ApoB, Lp(a), blood pressure, glucose, smoking, family history) a clinician weighs together.

  3. 3

    Learn how statins work

Understand the mechanism and broad intensity categories, and the monitoring topics people often ask about.

  4. 4

    Explore PCSK9 science

Learn the biology, the established injectable options, inclisiran where authorized, and why oral agents are described as emerging.

  5. 5

    Prepare questions

Complete the worksheet on your own device and bring it to a locally licensed cardiologist.

Questions they might take to their local cardiologist

  • How does my calcium score fit with my other risk factors?
  • Is my current treatment achieving what you intend for me?
  • Are PCSK9-targeting therapies authorized and reimbursed here, and would I meet local criteria?
  • What is known so far about oral PCSK9 inhibitors, and are there registered trials locally?
  • Should my Lp(a) or ApoB be measured?
