Visit checklist

Bring copies of recent reports (CAC, lipid panels, ECG, imaging) to your own clinician.
List all medicines, doses, and supplements, including any you stopped and why.
Note family history of early heart disease, stroke, or very high cholesterol.
Write down symptoms you have noticed, when they happen, and what relieves them.
Write down your top three questions in priority order.
Ask what follow-up is planned and how results will be shared with you.
Ask how to reach the care team between visits and when to seek urgent care.

Questions for My Local Clinician

1. What does my CAC score mean for my overall risk?

2. Which of my risk factors matter most right now?

3. Is my current treatment meeting the goals you have for me?

4. Are other therapies, including PCSK9-targeting options, relevant or available locally?

5. Should Lp(a) or ApoB be measured if they have not been?

6. What lifestyle changes would make the biggest difference for me?

7. When should I repeat tests, and which ones?

8. Which symptoms should prompt me to seek urgent care?
