Hypertriglyceridemia
Nutrition-first management for mildly elevated triglycerides, with stepwise consideration of omega-3s and prescription therapy for higher elevations.
Nutrition-focused foundation
The lipid profile demonstrates a mild elevation in triglyceride concentration.
Nutrition-focused interventions may include reducing intake of refined carbohydrates and other rapidly absorbed carbohydrates, provided this strategy remains consistent with the patient’s overall nutritional requirements, functional status, and treatment objectives.
Omega-3 titration
Nonprescription omega-3 fatty acid supplementation may be considered, with gradual titration toward a combined daily dose of approximately 4 grams if tolerated.
When additional triglyceride reduction is clinically indicated, prescription omega-3 formulations such as Lovaza or fibrate therapy with fenofibrate may also be appropriate, provided there are no contraindications, clinically significant drug interactions, or prior medication intolerance.
Accurate LDL measurement
Marked triglyceride elevation may reduce the accuracy of calculated LDL cholesterol values. Under these circumstances, direct laboratory measurement of LDL cholesterol may provide a more reliable assessment for therapeutic decision-making.
